Registration Information
Once a player
has been notified of their selection to a team, they must
register as a member of the club and complete the required
paperwork to be added to the team's roster.
In order to
register to play on the Jersey United Club Teams, players and
parents must complete the following paper work: (forms are also
listed below)

1) Jersey United Club - Player Registration Form
The player registration form will allow the player to become a
member of Jersey United Soccer Club, and entitles the player to
all the benefits of membership to this Club. This form must be
completed and signed by both the player and the Parent / Legal
Guardian.

2) Medical Release Form
The medical release form must be completed by the parent / legal
guardian and then signed while being witnessed by a notary
public.

New Jersey Youth Soccer Membership Form
This form must be completed and signed by both the player and
parent

US Club Soccer Registration Form
This form must be completed and signed by both the player and
parent

5)
Copy of Player's Legal Birth Certificate and Photograph
Copy of original birth certificate (or equivalent international
document) plus a small (1 inch square ) recent photo of player's
face for use on player passes.

6) Fees
Complete and return the proper fees as outlined on the
appropriate Jersey United Club Player
Registration Form (See # 1 above).
